Clinical Assessment

S: Owner reports polyuria/polydipsia × 3 days, decreased appetite, lethargy. No vomiting or diarrhea. Indoor only. UTD on vaccines.

O: QAR, BCS 4/9, MM pink and moist, CRT <2s. Mild dehydration (5%). Kidneys small and irregular on palpation. HR 168, RR 24, T 38.5°C.

A: Azotemia (BUN 45, CREA 2.8) consistent with chronic kidney disease. IRIS Stage 2 suspected. USG pending for confirmation.

P: Start IV fluids (LRS @ 60ml/hr), monitor UOP, recheck chemistry in 24hrs. Consider renal diet transition. Owner education on CKD management.
